Description:

• Provide comprehensive nutrition assessment and treatment for participants as part of an interdisciplinary team.
• Develop and implement individual and group nutrition services.
• Ensure integration into the care plan and monitor participant progress.
• Travel to various locations to deliver care and participate in team meetings.

Key Responsibilities:

• Perform assessments and evaluate nutritional status using professional guidelines.
• Calculate therapeutic diets tailored to individual needs and recommend care plans.
• Maintain participant files with diet orders, modifications, and assessments.
• Provide nutrition counseling and education to participants and caregivers.
• Coordinate with interdisciplinary team to ensure comprehensive care plans.
• Travel to participant locations to deliver direct care and conduct meal observations.
• Develop activities, diet groups, and educational programs with the Nutrition Services Team.

Requirements:

• Bachelor's Degree from an accredited Dietetics program.
• Minimum of one year of experience in clinical nutrition.
• Experience working with frail or elderly populations.
• Home care experience preferred.
• Current registration as a Registered Dietitian Nutritionist (RDN).
• Certified Dietitian Nutritionist (CDN) license in New York.
